Little Lake, Meadowlark , James

PROFILE OF RESIDENTIAL SERVICES

Each residential location is a separate program; however, all programs share resources and are managed by the Summit Human Services Inc. Simcoe management team. We have included a brief description of each home and will continue to update this as our resources expand.

LITTLE LAKE HOME PROFILE

Location: 9 Little Lake, Barrie, ON.

Staff:

Staff operated-rotational staff made up of Primary Youth Workers and Support Youth Workers.  Support staff vary depending on the numberof residents in the home and on the programmed activities and appointments. Full time in-house Team Leader working on site.

Physical Description:

This is a large 3200 sq. ft. two-story brick house situated on a very large treed lot.  There are five single bedrooms and a private master bedroom for the Houseparent.  The home has a large foyer, staff office, bathroom, dining room, kitchen, living room, and t.v. room, and large staff office, are all on the main floor.  The residents’ bedrooms are all upstairs, where there is also a full bathroom and a games room. The basement is currently unfinished and will currently be used for its laundry room and for storage.  A rec. room has been build downstairs and plans are to also include a full bathroom.

Curriculum Vitae:

All staff have graduated from post secondary education programs in various human services fields such as Child and Youth Worker, Social Services and Police Foundations. All staff are female and have been certified in Crisis Intervention. Many staff have also received additional training in: autism spectrum disorder, FASD, attachment disorder and bonding and positive discipline .

Child Care Support:

RYW provide both support and relief. Support staff may be male or female depending on the residents’ needs and risk levels. All staff are certified in Therapeutic Crisis Intervention.

Placement Preference:

Teenagers between the ages of 12 and 16 years of age who can benefit from a
relationship based, structured and consistent environment.  Difficult to serve, socially, behavioural, and challenging teens that can function within a community-based environment may benefit from the program.  Teenagers must be in school or working to promote responsibility and a sense of accomplishment.  We are not able to provide care for Young Offenders serving custody or detention orders.  We are also unable to provide care for known sexual offenders, fire setters, or extremely violent/ assaultive teenagers; however each case will be assessed individually.

Recreation Facilities and Activities:

Household activities include sports and recreation such as: skating, sliding, soccer, swimming, and hiking.  In house activities include board games, cooking lessons, videos, computer, and Nintendo. The area is academically well serviced with ongoing communication and support between the schools and home.  The home is close to a hospital and a short drive to shopping and community centers.

General Overview:

The home’s emphasis is on teaching teenagers how to develop healthier lifestyles with an emphasis on: relationship building, hygiene, nutrition, financial responsibility, school, community awareness, peer groups and positive age appropriate activities.

 

Location: 40 Meadowlark, Barrie, ON

Staff:

Type of Home:

This home’s program best serves youth between the ages of 12 and 16 years of age, who are in need of short-term or long-term placement.

Physical Description:

This is a large 3000 sq. ft. brick house with five bedrooms, a staff office, a huge kitchen, a breakfast nook, a living room, a beautiful family room, 2.5 bathrooms, a den, a laundry room and a walkout basement. The backyard is fenced and has a garden.

Curriculum Vitae:

All staff have graduated from post secondary education programs in various human services fields such as Child and Youth Worker, Social Services and Police Foundations.
All staff are female and have been certified in Crisis Intervention. Many staff have also received additional training in: autism spectrum disorder, FASD, attachment disorder and bonding and positive discipline.

Child Care Support:

Support workers are in the home daily, four to eight hours a day, depending on the number of residents in the home and on the programmed activities and appointments.

Placement Preference:

Female pre-teens and teens between the ages of 12-16 years of age, with emotional, behavioural and social needs.  We are not able to provide care for Young Offenders serving custody or detention orders.  We are also unable to provide care for known sexual offenders, fire setters, or extremely violent/ assaultive teenagers; however each case will be assessed individually.

Recreation Facilities and Activities:

The home is located near shopping centers, a community center and a pool.  The home’s fenced yard is next to a large park.  Program activities emphasize age appropriate social outings.

General Overview:

This is a well structured, reality-based home capable of handling children with difficult behaviours, emotional and social needs.  The home’s program emphasizes healthy boundaries, healthy relationships with adults, positive peer skills and age appropriate behaviours.

JAMES HOME PROFILE

Location: James Street, Barrie, ON L4N 6X9

Foster Parents:

Henick Foster Family

Type of Home:

Therapeutic Foster Home with capacity for 2 children- boys up to 16 years of age or girls-up to 14 years of age with emotional, cognitive, behavioural and social impairments.

Physical Description:

Large 4 bedroom home (2 bedrooms designated for foster children), 2 full bathrooms, large eat in kitchen, 2 separate living room areas, office and finished basement which has an additional bedroom in it. Laundry facilities are located on the main floor. The home has a large fenced yard and 2 car garage. A separate entrance leads to a full independent apartment which is rented privately.

Curriculum Vitae:

The foster parents are both graduate from post secondary institutions, have several years of work experience in various sectors and have experience as surrogate caregivers for international students and have had lots of interaction with young adults.

They have been trained and are certified in Therapeutic Crisis Intervention (TCI), attend quarterly re-fresher training, and are completing the Daniel Memorial Institute Foster Parent Training System.

The foster parents have two teenagers of their own and look forward to providing youth with opportunities for change and growth.

Child Care Support:

Trained Residential Youth Workers provide relief and support to the Foster Parents as needed, based on the occupancy rate. All Residential Youth Workers have successfully completed Summit’s Orientation training, In-House training, have received training and certification in Therapeutic Crisis Intervention (TCI) and attend quarterly mandatory re-fresher training. Training is also on-going in specialized areas such as FASD.

Placement Preference:

Youth, up to the age of 16 for boys, or age 14 for girls, with emotional, behavioural and social needs, who can benefit from a structured but warm and caring environment. Difficult to serve, socially, behavioural, and emotionally challenged youth who can function within a family model and community based environment. We are not able to provide care for Young Offenders serving custody or detention orders. We are also unable to provide care for known sexual offenders, fire setters, or extremely violent children; however each case will be assessed individually.

Recreation Facilities and Activities:

The home is located in the Central, West end of Barrie. They are close to all amenities and have access to hiking/nature trails, boardwalk, parks and schools, around their home. The foster family uses the Holly Recreation Center, and various golf courses in the area. Their children are involved in different activities in the community such as dance, soccer and hockey.

Clinical Support Services:

Dr. Amber Filiatrault, a consulting Psychologist, meets with Summit Human Services Simcoe County monthly to review Plan of Care goals, review assessments and recommendations, assists with program (in-house) assessment tools to measure and track outcomes and is available on a fee for service basis for individual assessments.

General Overview:

Organized, structured, well maintained family model home which promotes growth by encouraging good communication, problem solving and life skills. There is a strong emphasis on building good relationships, honesty, responsibility and respect towards each other. Personal accountability, consistency, and goal setting are stressed for those lacking self-direction.
